Tell me about a time a production incident occurred despite your testing. What did you learn?

  • production incidents
  • root cause analysis
  • continuous improvement

Answer it first, then compare. Nothing here is saved or scored.

Model answer

Show accountability without self-blame. Describe the root cause analysis, what the test gap was, why it existed (e.g., untested integration path, environment difference, data edge case), and what systemic improvements you made. Strong answers show you drove post-mortem action items: added monitoring, expanded test coverage, or improved environment parity.

What interviewers listen for

  • Ownership
You name what you personally did, not what "the team" did.
  • Calibration
You can articulate when you escalated, and when you absorbed the issue without escalating.
  • Influence without authority
You moved a developer or a PM by sharing data, not by pulling rank.
